In this paper, we will discuss an architecture of information system which enables “questionnaire schema sharing” about the problem domain of school evaluation. From our fundamental analysis, its basic requirements are minimizing cost of installation and operation of the system, and independency of specific questionnaire media form, which we have implemented. Next, we will discuss about our questionnaire form XML editor, which is based on reusability of questionnaire schema and diversity of questionnaire schema provider. In addition, by showing its data specification, we will talk about how we designed the reusability and diversity of meta questionnaire schema, and how we expanded it. As for the evaluation of our research, we will describe qualitative analysis of reusability of questionnaire schema and diversity of questionnaire schema provider, then we will show quantitative analysis of questionnaire administration cost. We will illustrate the usability of our system by reporting several cases in actual school evaluation. Finally, we will show our uniqueness by comparing cases that share the same problem domain or the same purpose.
